Thats not pain.. The reation isnt sever enough for the sudden shock for pain. That looks to me to be deep intense passionate desire, bordering on lust.

I'd say, someone just kissed her shoulder just softly enough for her to get goosebumps. That is what that look is to me. And that's a very nice feeling...LMAO!! The lights!
