Attention to the Background is imporant. When positioning your subject, look for distractions in the background. Careful attention was paid to the position of the water spraying out of the fountain, as well as what would make her head standout, etc.

Could not agree more. While we can debate the exact percentage, my estimate has always been the background (and/or middle ground) is 40% of a scene. Throwing it in as an afterthought can only result in a crippled image and a poorer viewing experience. IMHO. :-)
It's all about not distracting from the main subject. I get a kick out of some photographers who think it's all about the rule-of-thirds. (I like to ask, "Oh, what about the rule-of-fourths?" LOL) But, it is entertaining to see photos where the background makes it look like something coming out of the subject's ear.
