It should be pointed out that all fields in the form must be filled out in order for it to submit properly. Even if you choose an available application in the list for the first question, the "other" description field must contain at least a keystroke of some kind to successfully submit the form, or it will force you back to fill out all questions after submission. This could potentially confuse some people who believe they've filled out all the necessary info.

Luxxeon is correct. I filled out the form moments ago, and could not submit until I entered something into the first description box, which is supposed to only be filled out if you pick "other" from the list of applications. The text instructions above the box indicate you should fill it out only if you pick "other" from the list, to clarify which software you use. Obviously, I chose 3dsmax from the list of available applications, but still had to enter information into the box in order to submit my answers.
