Your start position in frame 1 should be in the total T-Stance without limbs poking through the skirt.  The last frame of your simulation should be the final pose of your character.  If you follow the instructions outlined in the comments above concerning constraining the garment, the simulation should run from start to finish with the desired result.  There should be enough give of the skirt to allow proper draping without tearing the mesh.
